MAMMOGRAM SCREENING SIGNUP!!
WHO CAN SIGN UP?
- WOMEN OVER Age 21 and OVER
- Age 40+ with no breast health concerns.
- Age 21-39 with no breast concerns but a family history of breast cancer
APPOINTMENTS
- Anytime between 11:00am and 3:30pm
WHAT TO BRING
- Patient should arrive 15 minutes early to complete forms.
- Valid Health insurance
- Valid Govt. ID or Passport
- Referring Provider Info
- Prior Mammogram Info
